American Training Center, in partnership with Elmwood Hills, has made special arrangements with NCBAC™ to make it possible for you to acquire your Certified Alzheimer Caregiver (CAC™) credential. Your participation in the following surveys is your “payment” for the exam.

PLEASE BE ASSURED THAT NO ONE WILL SEE YOUR INDIVIDUAL ANSWERS. YOUR RESPONSES ARE DIRECTLY ENTERED ON THE NCBAC™ WEBSITE AND WILL BE IMMEDIATELY CODED WITHOUT ANY REFERENCE TO YOUR NAME.  RESULTS WILL ONLY BE SHOWN FOR THE GROUP.
